Men with hypogonadism show fatigue, muscle loss, mood changes, and decreased sexual function. Younger adults occasionally present with premature hormonal decline from stress, autoimmune conditions, or prior surgical intervention.

The clinical standard, consistent with Menopause Society and Endocrine Society guidance, is that symptoms plus laboratory confirmation together justify initiation, not symptoms alone. A woman who reports hot flashes but shows estradiol within normal range for her cycle phase deserves a different workup, not a hormone prescription. In women under 45, FSH testing is particularly important for distinguishing ovarian failure from other causes of hormonal disruption before initiating therapy. Document both the symptom burden and the laboratory confirmation before writing anything.

Absolute and relative contraindications

Absolute contraindications require a hard stop. Do not prescribe BHRT to patients with a personal history of breast cancer, active hormone-sensitive malignancy (prostate, ovarian, endometrial), history of VTE, DVT, or pulmonary embolism, prior stroke or MI, active liver disease, current pregnancy or breastfeeding, or undiagnosed vaginal bleeding. These are required stops regardless of how compelling the symptom picture is.

Relative contraindications require individualized risk-benefit analysis: strong family history of breast cancer, migraines with aura (particularly with oral estrogen), uncontrolled hypertension, triglycerides above 400 mg/dL, thrombophilic disorders such as Factor V Leiden, and severe untreated sleep apnea in men being considered for testosterone. For patients with any clotting risk factor, transdermal estrogen is the preferred route. Oral estrogen carries meaningfully higher VTE risk compared to transdermal preparations and should be avoided in this subset.

First visit intake: what to gather before labs

A thorough history at the first visit shapes both your lab order and your route selection. Collect menstrual history, surgical history including hysterectomy and oophorectomy, prior hormone use and response, current medications, especially anticoagulants and hepatically metabolized drugs, and family history of hormone-sensitive cancers. Women who have had a hysterectomy don't need progesterone for endometrial protection, which changes the prescribing framework significantly. Get this information before you order a single lab.

BHRT Prescribing Protocols: Baseline Labs

The hormone and sex-specific panel

Core hormone labs include estradiol, total and free testosterone, SHBG, DHEA-S, progesterone if the luteal phase is clinically relevant, FSH, and LH. In women under 45 where menopausal status isn't clinically clear, FSH and LH are essential for confirming ovarian failure versus other causes of hormonal disruption. Before initiating testosterone in men, PSA is a baseline requirement, not optional. Before starting estrogen in women, confirm that mammography is current.

Cardiometabolic, thyroid, and general health markers

Every patient starting BHRT needs a CBC, CMP, fasting lipid panel, fasting glucose or HbA1c, and hs-CRP. The CBC gives you a hematocrit baseline, which is critical before testosterone. The CMP covers liver and kidney function, both relevant for metabolism of compounded hormones. Add a full thyroid panel: TSH, free T3, free T4, and TPO antibodies if autoimmune thyroid disease is on the differential. Undiagnosed hypothyroidism mimics hormone deficiency symptoms, and thyroid dysfunction alters sex hormone metabolism in ways that can make BHRT appear to be underperforming.

Vitamin D and ferritin are worth including in the baseline panel. Deficiencies in both are frequently observed in functional medicine patients, may affect symptom response to hormone therapy, and are straightforward to address concurrently. Missing them early creates confusion about why a patient on an adequate hormone dose isn't improving.

Why baseline labs matter more with compounded formulations

Compounded BHRT carries real potency variability. Published data shows compounded estradiol can run 26% below label strength, while compounded progesterone can come in 31% above. Without a serum baseline and follow-up levels, you're adjusting doses based on guesswork. This is why the Menopause Society and Endocrine Society recommend FDA-approved bioidentical hormones as first-line, with compounding reserved for cases where FDA options don't fit due to allergy, dose mismatch, or unavailability. BHRT Prescribing Protocols, Dosing and Delivery Methods

Estradiol starting doses and BHRT titration schedule

Starting doses by route give you a practical framework for the first prescription. Use these as a structured reference for your initial BHRT prescribing protocol decisions:

  • Transdermal patch: 25, 50 mcg/day
  • Transdermal gel: 0.75 mg/day (1 pump)
  • Oral micronized estradiol: 0.5, 1 mg/day for menopausal management
  • Sublingual: 0.5, 1 mg/day
  • Subcutaneous or IM injection: 2, 4 mg/week of estradiol valerate or cypionate

Check serum levels at 8, 12 weeks and target 40, 100 pg/mL for menopausal symptom management. Increase in one-step increments only when symptoms persist and levels are below target.

Feminizing therapy doses run 2 to 4 times higher, targeting 100, 200 pg/mL. For that clinical goal, 2 mg oral estradiol may be subtherapeutic. Flag this distinction in your charting to avoid undertreatment.

Progesterone dosing for endometrial protection

Micronized oral progesterone is the standard of care for endometrial protection: 100 mg nightly on a continuous schedule for postmenopausal women, 200 mg nightly sequentially on days 1, 12, or 1, 14 for perimenopausal patients. Compounded topical and vaginal progesterone preparations lack sufficient clinical evidence for endometrial protection and should not be substituted for oral micronized progesterone in this context. Women who have had a hysterectomy don't require progesterone. If you choose to include progesterone for other clinical reasons, document that rationale explicitly in the chart.

Testosterone dosing across populations and routes

In women, start low to avoid virilization. For topical testosterone gel, evidence-based starting doses are typically 20, 50 mg/day (approximately 1, 2 pumps of a 1% gel), with a serum target in the mid-normal female range of roughly 50, 150 ng/dL. Patch-based delivery typically starts at 2, 4 mg/24 hours. In men, 50, 100 mg/week subcutaneous or IM cypionate or enanthate is a standard starting dose, with adjustments based on 8, 12 week trough levels.

Pellet therapy carries the highest risk of supraphysiologic levels across all delivery methods and has no standardized dosing equivalents against FDA-approved preparations. Evidence shows pellets can produce testosterone levels well outside physiologic ranges, with no reliable way to reverse the dose once implanted. If you offer pellets, build in more frequent early monitoring and document the informed consent discussion thoroughly.

Writing a complete BHRT prescription

Required prescription elements for BHRT

A BHRT prescription that travels cleanly to the compounding pharmacy and stands up to chart review includes specific elements in every case: the full chemical name of the hormone (17β-estradiol, micronized progesterone, testosterone cypionate), exact dose per unit, frequency and timing, route of administration, cycle days if the regimen is cyclic, total duration, and refill instructions. Vague language creates errors. "Low dose as needed" is not a prescription; it's a liability document waiting to happen.

Compounding notes and labeling instructions

When ordering a compounded preparation, specify the base formulation (hydrophilic cream, sustained-release capsule), concentration (for example, 10 mg/g cream or 100 mg capsule), total quantity (60 g of cream, 30 capsules), and storage requirements. The patient-facing label needs to include the application site, amount per application, rotation instructions, and cycle timing if applicable. A label that reads "apply daily" is not sufficient for a patient using a testosterone cream on rotating sites with a cyclical progesterone schedule.

Titration language that travels with the prescription

Build explicit titration instructions into the prescription note or patient chart. For testosterone, an example titration note might read: "Increase by 25 mg if symptoms persist after 8 weeks and serum trough levels are below 80 ng/dL." This kind of language protects you and guides the follow-up provider. The specific increment and threshold should be adjusted based on individual clinical context and relevant guideline recommendations. Add informed consent documentation confirming the patient understands that compounded preparations lack FDA approval and that serum monitoring is required as part of the program. Both elements belong in the chart at initiation.

BHRT Prescribing Protocols: Monitoring and Safety

The 6, 12 week follow-up protocol

At 6, 8 weeks after initiation or any dose change, repeat the relevant hormone levels, check hematocrit and hemoglobin if testosterone is in the protocol, review blood pressure, and conduct a structured symptom assessment. The clinical rule on dose adjustment matters here: adjust only if symptoms persist and labs confirm levels outside the target range. Don't chase numbers at the expense of the clinical picture. A patient who feels well at a testosterone level of 70 ng/dL doesn't need her dose increased because the number isn't at the top of the range.

Annual monitoring once the patient is stable

For women on stable BHRT, annual monitoring includes mammography, pelvic exam if applicable, full hormone panel, lipid profile, CMP, and CBC. Consider bone density at appropriate intervals based on age and risk. For men, PSA monitoring every 6 or 12 months for the first two years is standard, with hematocrit every 3 or 6 months in year one, then annually, along with testosterone trough levels and a lipid panel. At every annual visit, screen for new contraindications: new cancer diagnosis, clotting events, new medications that affect hormone metabolism.

Red flags that require immediate clinical action

Before the first prescription is written, patients need to know which symptoms require same-day evaluation: leg pain or swelling, sudden chest pain or shortness of breath, severe headache, vision changes, speech difficulty, and unexplained vaginal bleeding. These aren't optional counseling points. Document them in the chart at initiation and again at each annual review.

Hematocrit above 52, 54% in men on testosterone requires clinical action. The Endocrine Society threshold for discontinuation is 54%. At that level, hold or reduce the dose, evaluate for secondary causes including sleep apnea, and restart only when hematocrit drops below 50%. Erythrocytosis is the most common testosterone-related adverse effect that requires active intervention, and it's preventable with consistent monitoring.
